To apply for Head Start or Pre-Kindergarten services, you may:

  • Visit our office at 1812 Welsh
  • Call (979) 764-5423 to request an application by mail
  • Contact sjackson@csisd.org to request an application by mail

The application packet consists of:

  • Child and Family Information
  • Language Survey
  • Special Needs Request
  • Parent Interest Survey

The following documents are necessary for the application process. Copies may be made at the office:

  • Birth certificate or passport. (If your child was not born in the United States, please provide a copy of immigration status)
  • Immunization record showing current immunizations for age:
    • DPT (Diptheria, Pertussis, Tetanus): 4 doses required, one dose due on or after 4th birthday
    • Polio: 3 doses required, one dose due on or after 4th birthday
    • MMR (Measles, Mumps and Rubella): 2 doses required, 2nd dose due on or after 4th birthday
    • HiB (Haemophilus Influenzae type b) vaccine: 1 dose required on or after 15 months of age, unless primary series and booster have been completed
    • Hepatitis A: 2 doses required
    • Hepatitis B: 3 doses required
    • PCV: 1 dose
    • TB (Tuberculosis): Skin test and result required or must pass TB screen
    • Varicella

  • Current Physical Exam (within one year)
  • Current Dental Exam (within one year)
  • Social Security Card (If a citizen or resident of the United States)
  • Verification of current family income. Bring all items listed below that apply:
    • Two most current pay statements for each parent employed
    • Assistantships / Pell grants
    • Funds received from country of residence
    • Assistance paperwork showing receipt of food stamps, TANF (Temporary Assistance for Needy Families), SSI (Supplemental Security Income), HUD (Housing for Urban Development), Medicaid, WIC (Nutritional Assistance to Women, Infants and Children), or Child Support
