Annual Routine Health & Counseling Services Consent
Starting Sept. 1, 2025, and in compliance with Senate Bill 12, in order for your child to be attended to by nurses, athletic trainers, and school counselors for routine health services like physical and mental health, parents must complete a Routine Health Services Consent Form for each child. Consent forms are available from school counselors and nurses or directly here.
Senate Bill 12
The Governor of Texas signed Senate Bill 12 into law on June 20, 2025. As part of SB 12, Texas public school districts are now required to seek consent from parents prior to providing any health-care services or health-related services to students. The law becomes effective September 1, 2025.
